At Pax Christi Hospice, we coordinate care
where the patient resides. Hospice fosters a different way of participating
in family life. You are in control of your health care. You can
decide to leave the program, and there are always opportunities
for hope. Remission of the disease can occur and people are
discharged.

To be eligible, a patient
should:
- Live on
Staten Island
- Have an
advanced illness, including non-cancer diseases, with the possibility of
limited life expectancy.
- Choose to
forego dramatic therapy, fewer invasive procedures
- Desire to
focus on correcting manageable symptoms with an emphasis on patient
preferences
- Have a
physician who is available and willing to manage care with the hospice team
- Desire to
be cared for at home.
Goals of Care
- Provide
intensive management of the symptoms or shortness of breath and pain
- Eliminate
unnecessary visits to the emergency department and admissions to the hospital
- Offer 24/7
accessibility to answer questions and promote patient comfort
- Provide
medications, supplies and oxygen related to the illness
- Schedule
intermittent visits from home health aides, nurses, social workers and
pastoral care
- Provide
clear explanations about the causes of symptoms
- Recommend
actions that will offer relief, reducing family anxiety and
stress
Pax Christi
Hospice is licensed by the New York State Department of Health; accredited
by the Joint Commission on Accreditation of Healthcare Organizations; and
approved for participation in the Medicare and Medicaid
Programs.

Payment for Services
Hospice will
bill the insurance companies of patient who have coverage for hospice
services. We will initiate a financial resource assessment for those who
do not have coverage. Co-pays and deductibles continue to be the patient's
responsibility.
