Senior Health Medical Practice 275 Eighth Avenue - at 24th Street 212-463-0101
St. Vincent's Senior Health Medical Practice is easy to reach by mass transportation and has a physician on call 24 hours a day. The practice delivers comprehensive care to older people and it treats them with dignity and understanding. Staffed by board certified physicians who specialize in geriatric medicine, the treatment team also includes psychiatrists, nurse practitioners, nurses and social workers, all with expertise in caring for seniors. In addition to offering excellent primary geriatric care to its own patients, Senior Health Medical Practice team members are available for consultations with patients' own physicians. Geriatric consultation and assessment are helpful in developing treatment plans to meet the needs of medically complex, frail elderly patients and their families. After consultation, the Senior Health Practice shares the information with the referring physician.

- "Welcome to Medicare" Examinations
|

Medicare covers 80% of its approved amount for a one-time medical examination during the first six months after enrollment in Medicare Part B, regardless of age. The "Welcome to Medicare" exam offers an important opportunity to review health status with a geriatrician, receive preventive services, including screenings, shots, and referrals to other care if needed. This exam serves to identify problems early, when they are generally easier to treat, resulting in far fewer health care problems.
The United Hospital Fund has created a website to help family caregivers and health care providers to work more effectively together to ensure safe and smooth transitions between care settings. Please click here to access this very useful site.
