Portfolio Requirements

- Completion of personal Goals
and Objectives form before each rotation.
- At least one self-critiqued
entry or self-reflection after each rotation in each competency section,
including level of success in achieving goals and objectives.
- Copy of evaluation from
webesprit inserted no longer than 60 days after the rotation is
ended.
- Self rating and attending
rating and signature of mentor or program director on competency grid
quarterly
- Copies of any presentations,
publications etc.
- Up-dated
