Responsible for the facilitation and co-ordination of the patient's care across the continuum, in collaboration with the multidisciplinary team, to efficiently manage all resources and improve quality of care. The Care Manager needs to be clinically astute and is considered a role model and clinical expert for nursing and other staff. Consistent with the need to analyze a patient's care holistically, the Care Manager must possess the ability to integrate all aspects of the patient's treatment while assessing the patient's needs and the adequacy of the existing care regimen, including the performance of all professional and support staff. The role focuses on the management of patient care, collaborating with the interdisciplinary team, and demonstrating effective management, leadership and communication skills with staff.
The Care Manager participates in the planning, implementation, management and evaluation of Quality Assurance and Improvement programs that enhance the quality of patient care.
The Care Manager directly supervises the professional and support staff assigned to his/her patients; evaluates their performance and skills at least annually; counsels and disciplines professional staff (up to and including suspension) for failure to provide proper care for skill-related matters; counsels and disciplines support staff (up to and including termination); represents the Hospital at the first step of the grievance procedure in professional practices, grievances or when otherwise designated by the Hospital.
