Position Description :
|
Pax Christi Hospice located in Manhattan is a unique program of care and support designed to meet the special needs of patients in an advanced stage of illness. The hospice has an inpatient unit and a home care unit. The home care program allows patients to remain with their loved ones in familiar surroundings. When appropriate, admission to the special hospice inpatient unit is available.
The hospice philosophy centers on support of patients and their loved ones. The goal is to relieve any pain, stress or discomfort that they may experience. The hospice team embraces caregivers as well as patients and encourages them to make choices that affirm their beliefs and maintain dignity. An interdisciplinary care team of skilled nurses, social worker, medical director, chaplain, volunteers and home health aides work together to provide holistic care ensuring physical, spiritual and emotional well-being to our patients and families at this very poignant part of the life cycle.
Job Summary: Provides spiritual counseling to patients and their families. Coordinates with religious community to meet patients' needs. Assists in accomplishing goals of anticipatory grieving.
Requirements: Graduate of C.P.E. program. Bachelor's degree. Two years of counseling experience. One year prior hospice experience preferred.
