As one of the nation's largest hospital based home care agencies, we are responsible for over 700,000 visits annually, serving all five boroughs as well as Nassau/Suffolk. Our programs are licensed by the New York State Department of Health and accredited by JCAHO. We provide skilled nursing, rehabilitative and social work services to New Yorkers in every stage of life. We offer home based infusion therapy, skin and wound care and home care for heart disease, asthma, orthopedics and neo plastic problems. An interdisciplinary team works together to provide the highest level of care to individuals who choose to remain at home.
Requirements:
New York State Occupational Therapist license and 2 years of Occupational Therapist experience in an acute care setting. Homecare experience preferred. Bachelor's required in Occupational Therapy. Masters' preferred.
