Position Description :
|
St. Vincent Catholic Medical Centers Early Intervention program located on Staten Island offers toddlers from birth to 3 years and infants as well as their family's early intervention to children who may have problems with a wide array of tasks. An interdisciplinary team works together to provide the child the services they need either in the home or in a day care center.
Job Summary: Under the supervision of the director, provides service coordination to eligible children and families following the rules and regulations set forth by the NYS Department of Health. The service coordinator is responsible for all activity related to policy and procedures regarding parents rights as outlined by New York City Early Intervention and SVCMC EIP, timeliness of service delivery, and communication with intra-and inter-agency staff, NYC Early Intervention, and community services.
Job Requirements: Associate's degree in Social Work or in the field of Health and Human Services. A minimum of 2 years experience in an approved Early Intervention Program, or a minimum of 2 years experience in the field of Health and Human Services. Excellent computer skills required.
