The Chelsea-Village Program of St. Vincent's Hospital has been providing quality health care to chronically ill and disabled elders of lower Manhattan since 1973. Our goal is to allow elders meet their own wishes to live in the comfort of their own homes at the best possible state of health and function and to avoid nursing home placement.
As one 92-year-old woman put it: "Thank God you're here. Please help me stay at home. I don't want to go to a nursing home."
The Living-At-Home Program (1987) and the Latinos Unidos en la Comunidad Program (2001) were established under the Chelsea-Village Program to provide improved care to Chinese- and Latino-Americans, respectively. Our bi-cultural and bilingual teams are fluent in English, Spanish, and many Chinese dialects. Click here to read it in Spanish and Chinese.

Who is eligible?
You are eligible for our services if you...
are chronically ill and homebound by reasonable definition
agree to accept our physician as the primary care provider
live in Manhattan below 42nd Street
are willing to accept us
What do we do?
We provide you with
-
a home visiting team made up of a doctor, a nurse, and a social worker
-
detailed geriatric-focused evaluations
-
team visits on a routine and as-needed basis
-
supportive counseling for family and caregivers
-
personalized care based on your goals
(At times, you may express a strong desire to avoid hospitalizations and focus more on being comfortable. We can work with you and your family to meet these goals.)
Based on eligibility and need, we make referrals to other agencies to provide you with

a hired caregiver
-
a visiting nurse
-
a physical or occupational therapist
-
laboratory tests, X-rays, EKGs, and ultrasounds at home
-
medical supplies and equipment
-
transportation to see specialists
-
financial benefits, including entitlements
-
patient advocacy
-
podiatry service
-
hospice care
What are the fees?
Chelsea Village Program services are free of charge for our patients.
How are we supported?
We are a not-for-profit agency. Beside Medicare fees, our financial support comes from our hospital and private donations. Everyone is welcome to participate in our exciting fundraiser events. Call if you would like to join our mailing list or would like to learn about ways you can help.
