Saint Vincent Catholic Medical Centers' Division of Continuing Care operates one of the nation's largest hospital-based home care agencies as well as three skilled nursing facilities and a hospice. 

Home Care
Our home health agency provides skilled nursing and rehabilitative services to thousands of New Yorkers in every stage of life where they feel most comfortable—in their own homes.  Home health services are available throughout all five boroughs of New York City as well as in Nassau and Suffolk Counties.  Hospice services are available through Pax Christi Hospice on Staten Island.

Skilled Nursing Facilities
We operate nursing homes in two boroughs, offering both short and long-term rehabilitation to patients who have suffered stroke, trauma, fractures and neurological and neuromuscular disorders.
