We admit residents to Bishop Mugavero based on a physician's order and in accordance with the resident assessment criteria and standards set forth by the New York State Department of Health. Submission of a completed Patient Review Instrument (PRI), screening form and financial information begin the assessment process. A pre-admission interview with the patient, physician and next of kin or significant other is required. We may also request additional medical or nursing information when necessary. All completed financial statements must be provided for submission to the New York State Medicaid Office, if applicable. A resident who currently receives Medicaid in the community is still required to provide documentation for long-term care coverage.
We review each application for admission on an individual basis to assess all relevant criteria, including the medical, nursing, social,and financial needs and concerns of the applicant. We accept patients for both short-term and long-term care.
How to Pay for Services Self-pay; Medicare (eligibility criteria); Medicaid; and private insurers with long term care coverage.
