Disability and Life Insurance Plans
At Saint Vincent Catholic Medical Centers (SVCMC), we understand how important comprehensive health insurance and financial protection is to our employees. With this in mind, we offer a comprehensive benefits program designed to meet the needs of our employees and their families.  We offer the following Benefit Plans and Programs for Non-Bargaining employees:

The Benefits described herein are for eligible Non-Bargaining employees only.  SVCMC offers similar benefits for employees in Collective Bargaining Units that are subject to the provision of the individual Collective Bargaining Units.  Therefore, benefits for Collective Bargaining Units are different than those described in these summaries.

Core Short-Term Disability Plan (STD)

SVCMC offers employees core Short-Term Disability coverage for non-occupational illness or injury.  For additional benefit protection employees may purchase Voluntary Supplemental Short-Term Disability benefits coverage.

Core Long-Term Disability Plan (LTD)
SVCMC provides eligible employees with Core Long-Term Disability coverage.  This benefit entitles the disabled employee, after 180 days of disability with a percent of their salary for disabilities resulting from illnesses or accidental injuries

Voluntary Buy-Up Long-Term Disability Plan
SVCMC offers a buy-up Long-Term Disability Plan that employees may purchase on a voluntary basis to increase their Core LTD benefit coverage. Employees may purchase this additional coverage through payroll deductions on an after-tax basis

Core (Term) Life Insurance and Accidental Death & Dismemberment (AD&D) for Employees
SVCMC provides employees with Core (Term) Life Insurance and Accidental Death and Dismemberment (AD&D) coverage based on the employee's annual salary up to a specified dollar limit.

Supplemental Life Insurance and Accidental Death & Dismemberment (AD&D)
Employees may purchase Supplemental Term Life Insurance and Accidental Death and Dismemberment Insurance in increments of one-times, two-times, three or four times salary up to a specified dollar amount.  Evidence of Insurability is required for certain coverage amounts.

Voluntary Group Variable Universal Life (GVUL) Insurance
Certain employees may purchase GVUL coverage of one-times to six-times their annual base salary.   Evidence of insurability is required for certain coverage amounts. This plan is portable and can be carried up to age 95.  There is also an investment vehicle component available under this plan.

Supplemental Term Life Insurance for Spouses and Children
Employees may purchase spouse or dependent child Supplemental Term Life Insurance provided they enroll in the Supplemental Employee Term Life Insurance benefit.

It is not intended to guarantee benefits.  SVCMC retains the right to modify or eliminate these or any other benefits at any time and for any reason.  If there is a discrepancy between this summary and the Plan Document, the Plan Document will prevail.
