The alternative to nursing home care:  Care at Home

Our Long Term Home Health Care Program makes it possible for chronically ill and disabled people - who might otherwise have no choice but to enter a nursing home - to continue to live at home, receiving the same services that they would receive in a nursing home.

Wherever you see this logo, it will be a reminder that staff members and physicians throughout Saint Vincent's Continuing Care Services are committed to a cooperative effort to provide person-centric care that we call "Becoming Home." This process is a way for all of us - patients, residents, family members, physicians, staff members - to work together to make our nursing homes more "home like" and our home health programs more sensitive to the cultural needs of those we serve. Experienced and Comforting  --
Home Care

Our Home Health Agency is one of the area's most experienced long term home care providers. We pioneered one of the original pilot programs in Manhattan in 1979, and today we provide skilled nursing and comprehensive care to some 1,000 patients in their own homes throughout Manhattan, Bronx, Brooklyn, Queens and Nassau County.

Many times, elderly or chronically ill patients who return home from a hospital stay are appropriate for the Long Term Home Health Care Program. This program provides necessary care in the setting where the patients are most comfortable - at home. Providing care in the patient's home minimizes the risk of infection. Generally, being home comforts patients and provides an incentive to improve.

Long Term Home Health Care  -- Designed for People Who Are:

  • Chronically ill or disabled, over 18 years of age
  • In need of monitoring by a registered nurse    
  • Under the supervision of a private doctor or clinic physician
  • In need of personal care and help with day-to-day activities
  • Covered by Medicaid or eligible for Medicaid coverage
  • Living in Manhattan, Bronx, Brooklyn, Queens or Nassau County

Referrals can come from physicians, hospitals, nursing homes, social workers or other healthcare professionals - as well as from patients themselves and their loved ones.

Personalized Care
Our first step is to evaluate your condition to determine exactly what services you need. We will work with you, your doctor and your family to design a plan of care just for you. We will arrange for a representative of your city or county human resources administration to visit you with us to get approval for your care. With human resources administration approval, we will provide you with the quality care you need for long term home health care Whether the care is for yourself, a loved one, or for your patient or client, we'll work closely with you and guide you through every step of the way.

Telehealth Home Monitoring
If you are living with a chronic condition like diabetes, congestive heart failure, chronic respiratory disease or some other complex illness, the new Telehealth system might be ideal for you. It's amazingly easy to use and allows your Saint Vincent's doctor, nurse and home care team to monitor your vital signs and symptoms every day, between personal visits.
