Rehabilitative Care

 

When a person experiences an acute illness or injury, he or she is admitted to the hospital, diagnosed, stabilized and a course of treatment is planned. In many instances, this course of treatment can be completed in a sub-acute center where the setting is more appropriate from a quality of life and cost effective perspective. At times, the physician may choose to admit directly to a sub-acute unit.

Our Clients Include:
People in need of Rehabilitation: People who can benefit from this service are those who have suffered from fractures or have elected to have an orthopedic procedure such as a joint replacement. Other candidates for rehabilitation are those who have had a CVA (stroke), neurological impairment, or who have had a lengthy illness and need to recover their strength before returning home.

People in Need of Skilled Nursing:
After hospitalization, many people need continued inpatient skilled nursing care to ease the transition back to home. St. Elizabeth Ann's provides patient education to help those who need to learn to manage their own care. We have successfully helped those in need of continuing:

• Respiratory care • Dialysis at an outpatient dialysis center
• Wound care • Diabetic care and education
• Intravenour therapy • Colostomy/ostomy care and education

The Team Approach
Members of the health care team include:

• Primary care physicians with training in internal medicine, geriatrics, neurology and family practice; a staff psychiatrist and psychologist are available as needed
• Nurse professionals
• Physical, occupational, and speech therapists
• A social work case manager
• Recreational therapist
• Registered dietitian
• Pastoral care
